Membership Freeze Request
Please complete this form to initiate your membership freeze.
Freeze Policy:
-Form must be complete 10 days prior to the end of the calendar month.
-No refunds for memberships that were not frozen prior to 10 days before the end of the calendar month.
-No refunds for memberships that were already charged.
-No freeze less than 30 days.
-Once frozen you will not be charge for your membership but will be charged for all left items on your tab.
